That’s why we’ve made our new patient forms available here on our website. Completing them before you arrive reduces the amount of time spent in our waiting room, and helps you get back to normal activities faster. WE ACCEPT YOUR INSURANCE

We understand how important insurance is to our patients. That’s why we’re in network for most PPO plans. We do our best to help you get the most from your plan, but whether or not we’re in-network with your provider, we’ll be happy to file a claim on your behalf.

We submit all insurance plans on behalf of our patients! It is our mission to provide you with affordable, high-quality treatment that fits your budget! We offer a variety of payment options to meet your needs. Our office accepts payments by cash, check and all major credit cards, including Care Credit. If you have any questions about financing or payment, please ask us! We will explain your choices and work hard to accommodate your needs.

We accept the majority of PPO insurance plans and will bill accordingly to maximize your benefits. Unfortunately, we do not accept HMO plans at this time. 

Please understand that your insurance policy is a contract between you and your insurance company and that our professional treatment is rendered to you, not the insurance company. Any problems of non-payment or delay of payment are the responsibility and obligation of you, the patient.

Please remember that dental benefits were never meant to determine your dental care. They are to assist the patient in the payment of your choice. Most policies cover what they consider a “usual and customary” fee. However, the insurance company establishes these fees to meet their needs and they are not always the same as the fees that may be charged in the office.

We will do our best to estimate what your insurance will pay for services. However, this is an estimate only. Some plans base the amount of benefit on a schedule of fees arbitrarily developed by insurance companies. For this reason, you may receive a lower percentage of the reimbursement than indicated in your dental plan. For example, if your plan states that it will pay 80% of the usual and customary cost per procedure, it means 80% of the fee determined by the insurance company and not the actual fee charged by our practice.

Please understand that we are not responsible for determining what your particular benefits are, but we will do our best to help you maximize your benefits. The ultimate responsibility for payment is yours and financial arrangements must be defined before treatment is rendered.

You are responsible for portions not covered by your policy on the day of service unless other prior arrangements have been made.

Any insurance balance over 45 days old is delinquent and is your responsibility to pay.

Accident insurance cases will be handled by the patient paying for the treatment at the time of service and your insurance will reimburse you.
