Simply Smiles - Family & Implant Dentistry is pleased to offer the Dental Membership Program to our patients not currently covered by dental insurance. Our annual Dental Membership Program allows for a complete continuation of preventative care. We want to help you prioritize your oral health. By providing dental discount plans, we can help our patients who are in between jobs or do not have insurance.

  • For patients with regular dental cleanings (no periodontal disease)
  • Routine Cleanings (two per year)
  • Routine oral exams (two per year)

  • 13% off all additional regularly priced services when paid by credit card on the day of service

PERIODONTAL
$695/year
  • For patients with periodontal (gum) disease
  • Periodontal maintenance cleanings (3-4 per year as determined by your dentist)
  • Routine oral exams by your general dentist (two per year)
  • Bitewing x-rays
  • One emergency exam per year (if needed)
  • 15% off all additional regularly priced services when paid by cash or check on the day of service
  • 13% off all additional regularly priced services when paid by credit card on the day of service

PROGRAM GUIDELINES

The Dental Membership is not insurance. The program is only offered to individuals and families not currently insured by a dental health plan. All members must receive their dental care at Simply Smiles - Family & Implant Dentistry. The program benefits are not transferrable to another dental practice or dental specialty practice. Payment is always due at time of service. Dental discount plans vary so please review this page for the full program and contact our office if you have any questions about the program.

 

  • It is the member’s responsibility to schedule and keep all appointments offered as part of the dental program.
  • Membership is for 1 year beginning on the enrollment date. Membership will automatically renew on the anniversary date unless a written request to cancel is received 30 days prior to the renewal.
  • Membership dues are payable in full upon enrollment and are non-refundable.
  • Cash, check or credit card payments are accepted.
  • Payments for additional dental services are the member’s responsibility. Payment is due on the date of service to qualify for the discounts detailed above.
  • Membership must be current to receive the discount.
  • Fees for dental services may change at any time.
  • Please notify our office at least 48 hours in advance if you must change a scheduled appointment.
  • No deductibles, no pre-authorizations, no yearly maximums and no waiting period.
  • No exclusions for elective care.
  • 30 days notice is required for patients who elect not to renew the membership program.
